Social Worker LCSW
Job Description:
A licensed clinical social worker (LCSW) generally possesses a master's degree in social work and has passed a state licensing exam. Licensed clinical social workers are mental health professionals who provide therapy, counseling, and intervention services to clients. LCSWs act as advocates and help clients gain access to resources while confronting their personal issues, such as mental illness, addiction, and abuse.
o Participates in the delivery of care as scheduled.
o Performs and documents patient's screenings, psychosocial assessments, and progress and referral information.
o Assists in the process of advocacy for patient's access to resources through community programs and coaching to the patients, when appropriate.
o Communicates treatment plan to patients, families and appropriate staff.
o Collaborates and coordinates with the caregiver, physician, medical assistant, director, service assessment, case management, utilization review, home health care and all other disciplines to provide the best care to patients.
o Provides support groups and individual counseling to patients on related psychosocial issues such as widowhood, coping skills, stress reduction, caregiver tips.
o Provides a level of comfort to the patient in showing them they are dealing with a well- trained professional.
Qualifications:
o Licensed in CA as a LCSW
o A minimum of one year of experience
